Job Summary:

We are seeking a detail-oriented and experienced Value Based Care (VBC) Medical Record Specialist to join our growing team. In this role, you will play a crucial part in ensuring accurate and efficient patient data management within our athenahealth EMR system.

Key Responsibilities:

  • Review external medical records to populate patient information in the Accompany Health athenahealth record.
  • Identify key evidence for quality gap closure and transfer data to designated locations.
  • Summarize patient trends and communicate findings to clinicians.
  • Categorize key documents and accurately identify documents within the AH EMR.
  • Identify HEDIS measure gaps and closures using the AH proprietary platform.

Qualifications:

  • Associate's degree or Medical Record Certification (AHIMA).
  • 3+ years of relevant work experience in healthcare with health information and/or direct medical record review.
  • Comfort with multiple technology platforms, including EMRs.
  • Strong attention to detail and accuracy.

Compensation:

  • $20-$25/hour (base salary range) + equity + benefits.
  • Salary range determined by role, level, and location.

Qualifications

Education:

  • Associate's degree in a related field or Medical Record Certification (AHIMA) required.

Experience:

  • Minimum 3 years of relevant work experience in healthcare, including health information management and/or direct medical record review.

Skills:

  • Proficiency with multiple technology platforms, including Electronic Medical Records (EMRs).
  • Strong attention to detail and accuracy.
  • Ability to identify key evidence within medical records and summarize patient trends effectively.
  • Familiarity with HEDIS measures and quality gap closure processes is a plus.

Key Responsibilities:

  • Medical Record Abstraction: Accurately extract and enter patient demographic, clinical, and treatment information from external medical records into the Accompany Health athenahealth electronic medical record (EMR).
  • Quality Improvement Support: Identify key clinical documentation supporting quality gap closure initiatives and transfer relevant data to designated platforms.
  • Trend Analysis and Reporting: Summarize patient health trends and communicate findings to clinicians to inform care plans and interventions.
  • Document Management: Categorize and accurately identify various medical documents within the Accompany Health EMR system.
  • HEDIS Measure Tracking: Utilize the Accompany Health proprietary platform to identify and track HEDIS measure gaps and closures.
