Job Description

We are seeking a passionate and experienced Social Programs Manager to lead the development and implementation of innovative social programs that address the social determinants of health for our diverse membership. This role will play a critical part in ensuring equitable access to care and improving the overall health outcomes of our community.

Responsibilities

  • Design, develop, and manage a portfolio of social programs focused on addressing social needs impacting health.
  • Collaborate with internal and external stakeholders to identify community needs and develop targeted interventions.
  • Build and maintain relationships with community partners and organizations.
  • Monitor and evaluate program effectiveness, making data-driven adjustments to optimize impact.
  • Manage program budgets and resources effectively.

Qualifications

  • Bachelor's degree in social work, public health, or a related field.
  • 3+ years of experience managing social programs, preferably in a healthcare setting.
  • Demonstrated understanding of social determinants of health and their impact on health outcomes.
  • Strong project management, communication, and interpersonal skills.
  • Passion for improving health equity and community well-being.

Qualifications

Required:

  • Bachelor's degree in Social Work, Public Health, or a related field.
  • Minimum 3 years of experience managing social programs, preferably in a healthcare setting.
  • Demonstrated experience developing and implementing community outreach initiatives.
  • Strong understanding of social determinants of health and health equity principles.
  • Excellent communication, interpersonal, and organizational skills.
  • Proficiency in Microsoft Office Suite and data analysis tools.

Preferred:

  • Master's degree in Social Work, Public Health, or a related field.
  • Experience working with diverse populations and underserved communities.
  • Knowledge of healthcare policy and regulations.
  • Project management certification.

Key Responsibilities

Program Development & Implementation:

  • Design, develop, and implement innovative social programs aligned with One Medical's mission and community needs.
  • Conduct needs assessments and research to identify target populations and program gaps.
  • Develop program budgets, timelines, and evaluation metrics.

Program Management & Oversight:

  • Manage day-to-day operations of social programs, including staff supervision, resource allocation, and vendor management.
  • Monitor program progress, track outcomes, and ensure adherence to established goals and objectives.
  • Identify areas for improvement and implement strategies to enhance program effectiveness.

Community Engagement & Partnerships:

  • Build and maintain relationships with community organizations, stakeholders, and potential partners.
  • Collaborate with internal teams to integrate social programs with One Medical's clinical and operational services.
  • Represent One Medical at community events and advocate for social health initiatives.
